Requirements

  • Bachelor’s Degree in a related field.
  • Excellent communication skills, both verbal and written.

Nice To Haves

  • Preferred experience with homeless or precariously housed populations.
  • Experience working with individuals with a behavioral health diagnosis preferred, but not required.
  • Bilingual Spanish is highly desired.

Responsibilities

  • Leading a team of staff throughout Manhattan to engage disconnected and homeless individuals into care by working with referral sources such as hospitals, primary care physicians, shelters, substance use treatment programs, mental health providers, SROs, and social service organizations, in addition to canvassing throughout Manhattan subways and streets.
  • Receiving and evaluating all referrals from community partners, the SOS HUB, and OMH to determine eligibility for services.
  • Serving as the primary liaison for all referrals to ensure potential participants are cleared for enrollment, and working with staff to ensure referrals are addressed in a timely manner.
  • Participating in community outreach on the streets and subways; coordinating participant needs before and after their transition from street to home; enhancing daily living skills; accompanying participants to appointments; and advocating on their behalf when faced with discrimination or healthcare inequities.
  • Building and maintaining connections with community-based providers to ensure eligible participants form meaningful relationships and connect to all appropriate services.
  • Completing and submitting all referral tracking documents to the Directors and CBC.
  • Providing accurate, complete, and timely documentation.
  • Working closely with the Directors of two SOS teams to ensure staff scheduling, task completion, and fulfillment of all Outreach & Engagement staff responsibilities.
