About The Position

At Stony Brook Medicine, our Social Workers are valuable members of our team, who provide empathic care to our patients, their families and visitors, with the highest degree of integrity and respect. Social Workers apply the principles and practices of their profession to provide integrated social casework, including direct counseling, therapy, advocacy, and other services to individuals, families and groups in a variety of settings. Social Workers plan, implement, coordinate and document treatment for individuals diagnosed with: mental illness and serious emotional disturbance; mental retardation and developmental disabilities; work-related illnesses or injuries; chemical addiction; or perpetrators or victims of neglect, abuse, or crime who need therapeutic intervention, habilitation, training, supervision and re-socialization, or confinement. Qualified candidates will demonstrate excellent communication skills, interpersonal skills, knowledge and understanding of patient care and effectively respond to changing patient needs by making decisions based on ethical principles and adhering to our high standard of excellence.

Requirements

  • MSW from an accredited Social Work Education School.
  • New York State LMSW or LCSW.
  • One to three years post Master’s.
  • Experience in medical health care setting or medical clinic setting.
  • Ability to implement and run support groups into required.
  • Demonstrates excellent written and oral communication skills.
  • Demonstrates strong Psychosocial Assessment skill set.
  • Demonstrates understanding of hospital systems, transition to care issues, social work with different patient populations.
  • Understanding of integrated health care model.
  • Working knowledge oncology specific care and resources.
  • Working knowledge of Long Island and Metropolitan NY Community resources.
  • Working knowledge of benefit and entitlement programs.
  • Demonstrates patient first, patient advocacy and service excellence.
  • Computer literate and understanding of data analysis.
  • Demonstrates the following: understanding of community systems; demonstrates ability for project management and action planning; demonstrates teamwork and team leader focus; understanding of data collection and statistics.
  • Demonstrates an ability to manage a large and complex case- load and an ability to work independently.
  • Demonstrates understanding of palliative care.

Nice To Haves

  • Three to Five plus years’ experience in direct clinical work with Oncology patients.
  • Experience with oncology support for all ages

Responsibilities

  • Psychosocial assessment specific to oncology patients
  • Assessment and follow up on oncology patients pediatric through geriatric
  • Coverage of outpatient cases upon admission to inpatient for continuity of care
  • Other outpatient coverage as needed or indicated
  • Counseling and Crisis Intervention
  • Coordinate with SB and other hospital discharge planning team for transition to care
  • Coverage and collaboration with Palliative Care outpatient initiative
  • Work with community agencies for ongoing oncology care
  • Coordination with insurance companies, insurance verification, advocacy with outside agencies.
  • Assist with access to care and address social determinants of health
  • Assist patient financial services, benefits/ entitlements or with Medicaid applications as needed.
  • Assist patient with grant applications for support
  • Coordinate and give review to insurance companies as needed; contact insurance companies for authorizations.
  • Coordination with CPS, APS, OMH, OPWDD, DSS, and other social service and oversight agencies.
  • Assist patients and families with advanced directives including MOLST if applicable
  • Supervision of professional social work students minimum every 2 years.
  • Distress tool screening for oncology patients with CoC compliance for standard
  • Active involved in performance improvement activities including program development, data collection and analysis of cases and follow up

  • Coverage of multiple clinic settings as needed including but not limited to; Adult Medicine, Cystic Fibrosis, OB/GYN, Geriatrics
