About The Position

NJ Palliative Care Solutions is hiring an experienced Nurse Practitioner or Physician Assistant to provide outpatient and skilled-nursing-facility palliative care across Somerset County and Central New Jersey. The role serves people living with serious illness in SNF, home, and community settings, including short-stay subacute and rehabilitation patients with recent hospitalizations, high symptom burden, or complex goals-of-care needs, as well as long-term-care residents. The position offers a structured ramp into palliative care, mentorship, clinical backup, data-guided coaching, clear workflows, and interdisciplinary support. The clinician provides holistic, patient-centered, evidence-based palliative medicine; manages complex symptoms; leads goals-of-care and advance-care-planning conversations; collaborates with patients, families, facility staff, primary care, hospice, and partner home-health agencies; and supports care transitions throughout SNF stays. Timely documentation must meet clinical, billing, and quality standards. Local travel within Central New Jersey is required, with mileage reimbursement and clinical/EMR support provided. This is a full-time position, primarily Monday through Friday, with flexibility to swap workdays. The employer is open to discussing part-time or per-diem schedules for the right candidate. Base salary is $130,000–$160,000 per year, with an annual performance bonus of up to 12.5% of base salary.
