About The Position

First Choice Health Centers is seeking a Nurse Practitioner or Physician Assistant for our primary care practice in East Hartford, CT. This position is accountable for independently performing a full range of tasks in delivery of direct patient care in an outpatient setting. Requirements

• Proven experience as a Nurse Practitioner or Physician Assistant establishing care plans and managing patient outcomes
• A current license to practice as a NP or PA in the state of Connecticut.
• Appropriate Board Certification
• A commitment to serving diverse and vulnerable populations with compassion and respect.
• Excellent communication, interpersonal, and organizational skills.
• Connecticut Controlled Substance license and DEA license
• BLS Certification through the American Heart Association or American Red Cross.
• Employees of First Choice Health Centers must be vaccinated against COVID-19. Certain exemptions may apply.
• Candidates are required to pass a drug test, including marijuana testing, before beginning employment.

Responsibilities

• Evaluates, assesses patient illness/injuries.
• Chronic care management
• Performs physical examinations, and special examinations.
• Takes medical histories.
• Develops plans of care.
• Provides routine and illness care.
• Orders medications; orders diagnostic tests; and evaluates results.
• Assumes responsibility for patient follow-up, coordination, and referrals to other health care providers as necessary.
• Maintains patient medical records.
• Provides patient counseling/education.
• Participates in medical student and health professional teaching.
• Performs related duties as required.
