About The Position

BAYADA Home Health Care is seeking an experienced Occupational Therapist (OT) for a part-time opportunity performing Part A and Part B Outpatient home health visits. This role serves adult and geriatric patients in senior living communities and group homes for adults with intellectual disabilities. The Occupational Therapist will be a key member of a multidisciplinary healthcare team, providing rehabilitative care and skilled nursing to clients, enabling them to receive necessary medical care while remaining in their homes.

Requirements

A current North Carolina Occupational Therapist license.
A minimum of one year of recent work experience as an Occupational Therapist.
Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA, as indicated by school transcript or diploma.
Ability to work independently and manage time effectively.
Strong interpersonal skills, organizational skills, and problem-solving skills.
Solid computer skills.

Nice To Haves

Prior home care experience preferred, but not required.
Prior experience with electronic medical records (EMR) preferred.

Responsibilities

Perform outpatient therapy evaluations to develop care plans for patients in designated senior living communities or group homes for adults with intellectual disabilities.
Practice at the top of licensure, utilizing current research-based approaches to maximize client functional independence.
Collaborate with home health therapy partners to transition care from home health to outpatient services, achieving client-centered goals within their living environment.
Educate clients, families, and community caregivers on care plans to foster a team approach for maximizing functional independence.
Conduct evaluations, treatments, and discharge planning, documenting individualized treatment approaches using the outpatient EMR platform.
