About The Position

BAYADA Home Health Care is seeking an experienced Occupational Therapist (OT) for a part-time opportunity performing outpatient home health visits. This role involves servicing adult and geriatric patients in senior living communities and group homes for adults with intellectual disabilities. The Occupational Therapist will be part of a multi-disciplinary healthcare team providing rehabilitative care and skilled nursing to clients, enabling them to receive medical care at home.

Requirements

A current North Carolina Occupational Therapist license.
A minimum of one year of recent work experience as an Occupational Therapist.
Graduation from an occupational therapy program accredited by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA.
Ability to work independently and manage time effectively.
Strong interpersonal skills, organizational skills, and problem-solving skills.
Solid computer skills.

Nice To Haves

Prior home care experience preferred.
Prior experience with electronic medical records (EMR) preferred.

Responsibilities

Perform outpatient therapy evaluations to develop a plan of care for patients in designated senior living communities or group homes.
Utilize current research-based approaches to maximize functional independence for clients.
Collaborate with home health therapy partners to transition care from home health to outpatient services.
Educate clients, families, and caregivers on care plans to foster a team approach for maximizing functional independence.
Perform evaluations, treatments, and discharge planning for clients.
Utilize the outpatient EMR platform to deliver and document individualized treatment approaches.
