Job Description Summary: To competently interpret the patients history and baseline level of function and then to complete an assessment appropriate to the patients areas of deficits. Based on problems identified, the physical therapist establishes and carries out a plan of care to include patient treatment and reassessment, patient/family instruction, discharge planning, documentation in the medical record and communication with physicians and other medical staff. This position helps patients achieve maximum functional outcomes. Acts as a clinical instructor for students and actively participates in department functions.

Requirements

  • Bachelor's Degree: Physical Therapy (Required)
  • Licensed in the state of Ohio.
  • CPR certification that is recognized by the American Heart Association.
  • Basic computer skills.
  • BS in Physical Therapy.
  • Licensed in the state of Ohio.
  • Entry-level new graduate, if facility has at least one veteran Physical Therapist to mentor, otherwise, at least one year of clinical experience.

Nice To Haves

  • Experience in the outpatient realm with focus for the neurologic population.
  • Additional Certifications for neuro evaluation and treatment such as LSVT BIG, NDT, CSRS, CBIS are beneficial but not required.

Responsibilities

  • Evaluates and treats patients: After provider referral, evaluates patient physical abilities and functional status, develops and implements plan of care based on evaluation findings and appropriate treatment techniques and initiates patient treatment. Carries out plan of care with reassessment and changes to plan of care as appropriate. Responsible for maintaining/meeting the productivity expectations of their department. Oversees and supervises provision of care by physical therapist assistants.
  • Plans and helps coordinate discharge plan in collaboration with the multidisciplinary team: Instructs patient and care giver/family. Recommends DME required for discharge. Provides information to providers, nurses, case managers, social workers, and other team members. Involved in the multidisciplinary patient plan of care and discharge readiness plan. Documents evaluation, plan of care, progress toward goals, discharge plan/continuity of care for multidisciplinary team, and other involved medical and insurance parties.
  • Supervises patient assignments and duties of the PTAs and rehabilitation tech(s).
  • Is involved in department quality improvement through data collection and participation in team and staff meetings.
  • Assumes duties delegated by management in the absence of department management.
  • Takes an active role in orienting new staff members.
  • Serves as clinical instructor for affiliating PT and PTA students.
  • Serves as an advocate for discipline on the multidisciplinary team.
  • Represents department at problem-solving and committee meetings.
  • As a High Reliability Organization (HRO), responsibilities require focus on safety, quality and efficiency in performing job duties.
