About The Position
We are looking for a Nurse Practitioner or Physician Assistant/Associate to join our dynamic, multidisciplinary team at our outpatient diabetes clinic. This role involves leading meaningful patient visits, crafting individualized care plans targeting glycemic control, disease prevention, and long-term wellness. You will partner with endocrinologists, diabetes educators, dietitians, and nursing staff to deliver holistic care, empower patients and their families with knowledge and confidence to manage diabetes through education on lifestyle, medications, and self-care, and order and interpret labs, fine-tune treatment regimens, and track outcomes. You will also contribute to quality improvement initiatives and clinic growth.
Requirements
- Nurse Practitioner or Physician Assistant/Associate with ability to obtain/maintain New Hampshire licensure
- National certification as a PA, FNP, or AGNP
- Clinical experience in endocrinology, diabetes care, primary care, or related specialty
- Strong clinical judgment, communication, and patient education skills
