Requirements
- Recent experience in diabetes patient education
- knowledge and comfort with facilitation of group patient education
- strong communication and interpersonal skills
- Registered Nurse
- Current NJ RN license
Nice To Haves
- BSN preferred
- Certification by the National Board of Diabetes Educators (CDE) preferred or eligible to take exam in 1 year
Responsibilities
- Counsel patients/families on diabetes self-management (either in 1:1 setting or in small group setting) incorporating ADA National Standards for Diabetes Self-Management Education and AADE adapted Behavioral Change Goals. Makes follow-up telephone calls and/or emails to patients as necessary.
- Documentation on patient records, EMR, Diabetes Database and written communication to patient’s medical provider(s) in accurate, timely, & professional manner.
- Provides diabetes wellness education via Virtua diabetes support groups, and community events.
- Participates in diabetes program & education curriculum development & revision.
- Serves as clinical diabetes resource by responding to requests for information & clinical updates from Virtua staff, physicians, schools, etc.
- Participates in new staff orientation and student intern observational experiences.
- Attends & participates in bimonthly staff meetings and other meetings as needed.
