About The Position

The Out Patient Counselor provides counseling to patients in an out-patient facility, who are chemically dependent, and develops a treatment program based on individual needs. The role will assist patients in developing an understanding of chemical dependency and facilitates effective methods to maintain a drug-free lifestyle.

Requirements

  • Provide counseling to chemically dependent patients.

  • Conduct substance use disorder screenings, assessments, and evaluations.
  • Complete comprehensive biopsychosocial assessments, substance use histories, and intake interviews.
  • Utilize DSM-5 and ASAM Criteria for level of care determination and treatment recommendations.
  • Coordinate and facilitate client admissions.
  • Develop preliminary treatment recommendations.
  • Coordinate referrals to various services and resources.
  • Provide crisis intervention and support.
  • Maintain accurate and timely client records.
  • Protect client confidentiality and privacy.

Responsibilities

  • Conduct substance use disorder screenings, assessments, and evaluations in accordance with program policies and procedures.
  • Complete comprehensive biopsychosocial assessments, substance use histories, and intake interviews.
  • Utilize DSM-5 and ASAM Criteria to assist in determining appropriate levels of care and treatment recommendations.
  • Coordinate and facilitate the admission process for eligible clients.
  • Develop preliminary treatment recommendations based on assessment findings.
  • Coordinate referrals to inpatient, outpatient, aftercare, mental health, medical, housing, employment, educational, legal, social service, and community resources.
  • Provide crisis intervention and immediate support for clients presenting with urgent behavioral health or substance use concerns.
  • Maintain accurate, complete, and timely client records in accordance with program requirements and professional standards.
  • Participate in multidisciplinary team meetings, staffing, case reviews, and treatment planning discussions.
  • Protect client confidentiality and privacy at all times.
