About The Position

To provide the necessary guidance, support, and therapeutic follow up that will enable each client to effectively utilize coping skills and resources throughout their time in Partial Hospitalization Program (PHP) to successfully complete and transition into lower levels of care.

Requirements

  • Associate or Bachelor's Degree related to the healthcare field. For Example: Social Work, Psychology, DARC program.
  • An active CASAC or higher is required.
  • Minimum 2 years Case Management experience in supporting clients with co-occurring disorders.

Responsibilities

  • Ensures client adherence to program expectations and is actively engaged in PHP programming through case management and additional therapeutic support.
  • Provides case management sessions, check-ins, and crisis support to support clients Clinical Wellness Plan.
  • Case Management sessions will include goal planning, support in developing a daily sober routine, support in attending appointments, developing a sober network, and developing an after-care plan based on clinical recommendation.
  • Will have knowledge of clinical, medical, and community supports in the local area to help clients engage in these resources to maintain overall well-being, support clinical wellness plan, and long-term recovery. Will need to continue to stay update on these resources and any new resources in the area.
  • As one of the client’s direct treatment team members will support weekly communication to family and community providers through meetings, phone calls, and e-mails.
  • Address day-to-day needs of clients while using trauma responsive interventions, maintaining a calm, welcoming and professional demeanor withing Mountainside’s standards and values.
  • Prepare to welcome new admissions by ensuring that all welcome materials are in place and blocked room and bed are clean and orderly according to established standards.
  • Complete admissions process, including greeting new clients and their families and completion of all necessary forms.
  • Orient new clients to facility, programs, and services.
  • Immediately communicate to Manager and Counselors any behavioral problems or safety concerns which may arise when working with clients.
  • Facilitate program groups and other activities as directed/needed.
  • Aid other team members as necessary to ensure that clients experience an unsurpassed level of service by taking initiative to support the recovery process
  • Directly manages scheduling of PHP clients in collaboration with the treatment team and OPS Administrative Assistant.
  • Helps clients identify sponsorship opportunities
  • Assists with arranging transportation for residents as needed for meetings, appointments etc.
  • Counsel clients individually and in group sessions to assist clients in achieving wellness plan goals.
  • Monitor condition of client to evaluate success of therapy and adapt treatment as needed
  • Support in the completion of FMLA, unemployment benefits, and/or disability paperwork.
  • Refer client to the appropriate level of care and assist client and family during this transition
  • Facilitate crisis interventions when necessary
  • Assist clients in facilitating a smooth transition back to work, home or school while carrying out Mountainside’s recommended continuing care plan
  • Function in a peer support role
  • Collaborate with client’s collateral supports in effort to meet client’s unique needs
  • Keep treatment team abreast to client updates and pertinent information
  • Prepare and maintain reports and case histories in Aura and Salesforce utilizing proper documentation techniques
  • Collaborate w/ the Continuing Care, CIT, Recovery Coaching and Alumni departments to ensure continuity of care
  • Develop, foster and maintain relationships with alumni
