Evergreen Treatment Services (ETS) is seeking an OTP Care Navigator to provide outreach, engagement, case management, and care coordination to adults facing behavioral health challenges, homelessness, and substance use disorders. This role operates primarily from an external agency site (ORCA Center/DESC) alongside ETS’s Outreach team, focusing on building trusting relationships to identify needs and goals, and offering ongoing support to enhance stability and quality of life. The position is fast-paced, requires multitasking, and demands exceptional interpersonal skills, with regular interaction with the executive team, employees, and external stakeholders. The OTP Care Navigator will be a key part of a multidisciplinary team, providing care coordination to a population often subjected to stigma. Team members are encouraged to foster a welcoming, non-judgmental environment that promotes harm reduction, improves engagement, and meets patients where they are. Services are delivered through a harm reduction lens and motivational interviewing techniques, with sensitivity to chronic, debilitating, or terminal health conditions. A primary responsibility is supporting participants transitioning from ORCA Center care by connecting them with suitable Opioid Treatment Programs (OTP) based on their individual needs, preferences, and recovery goals. This includes assisting with the New Patient Orientation group, monitoring and contacting new participants regarding missed medication dispensing appointments within their first 90 days to encourage engagement, and documenting these encounters in the participant's electronic treatment file while consulting with the treatment team. Collaborative duties include communicating with the Medical Provider regarding outside agency communications, registering prospective participants, coordinating and completing intake assessments, chaperoning telehealth equipment during intake with a remote Medical Provider, and coordinating with partner OTPs in Washington state, including transporting participants.
