About The Position

The Streetlight Program Assistant (OPS) herein supports the Streetlight program’s mission to provide age-appropriate peer support to chronically ill adolescents as a model of pediatric palliative care. The Streetlight Program Assistant assists with administrative duties, patient support, and volunteer support.

Responsibilities

  • Recruits, screens, and orients new volunteers
  • Reviews EPIC charts and filters patient census and medical information into a Streetlight team-member appropriate "need-to-know" format
  • Records daily patient referrals from medical staff and consolidates information into task-lists for team members
  • Manages Streetlight encrypted SharePoint site featuring weekly team educational readings, announcements, and educational web links
  • Manages REDCap database including transcription of patient demographics, diagnoses, and volunteer psychosocial chart entries
  • Facilitates communication and collaboration with interdisciplinary team members to best meet the needs of patients enrolled in Streetlight
  • Assists with new volunteer orientation and continued education in palliative care, chronic and terminal illness, HIPAA compliance and best practices to provide psychosocial care to the target population
  • Conducts project-based volunteer meetings
  • Organizes and archives team education materials
  • Coordinates team member graduation ceremony and other social events
  • Introduces Streetlight program to new designated patients
  • Provides psychosocial support to patients including inpatient room visits, outpatient appointment visits, and event planning
  • Provides end-of-life support including creation of legacy projects
  • Regularly coordinates activities for patients/families (milestone events, unique long-term patient events, etc.) including management of logistics, catering, and community involvement.
  • Organizes weekly meal deliveries for Cystic Fibrosis patients through the BJ’s CF Dining Days Program
  • Maintains Streetlight active patient database, used for program census
