Requirements

Experience working in a 120-bed skilled Long-Term Care facility
Knowledge of Medicaid/Medicare eligibility or insurance processes.
Must have excellent communication and interpersonal skills.
Must be fluent in the English language: reading, writing, speaking.
Must have basic skills in the use of Microsoft Office Outlook and Word.
Must be willing to work evenings and weekends as needed.
Must be a Florida Licensed Clinical Social Worker
Successful completion of the employment screening process to include, but not limited to, a national background investigation and drug test.

Nice To Haves

Bachelor’s degree.
Knowledge of Health Insurance Authorizations & Eligibility & knowledge of Medicare A.

Responsibilities

Provide management and coordinate case management, treatment, referred counseling and follow up services for residents.
Develop and implement policies and procedures for social services.
Perform duties as backup to the Admission Coordinator when needed.
Provides management, coordination and oversight for the social services department.
Monitors participation of the veteran resident in programs/services designed to meet psychosocial, mental and emotional needs of the veteran.
Participates in Interdisciplinary Treatment team meetings to develop a care plan.
Provide knowledge of Medicaid benefits to families and residents.
Perform other duties as assigned.
