About The Position

This position serves as the liaison for the Community Integrated Mobile Services for the DOH-Hernando. Primary duties include organizing and attending outreach activities (which may be after normal business hours or on weekends), maintaining and strengthening community partnerships, providing education to residents based on the community Health Needs Assessment (CHA) process, and attending of community meetings. Serve as the Department of Health Hernando County subject matter expert with community partners and at community presentations. The position is responsible as a “need to know” employee for security sets relating to active medical records as it relates to performance of assigned duties and responsibility of same. Responsible for securing the designated information set for the purposes of protecting confidentiality, data integrity, and appropriate access, for all information, both confidential and public record, which are stored in hard copy or electronic format(s).

Requirements

  • Knowledge of the principles and techniques of effective communication.
  • Knowledge of the methods of data collection.
  • Knowledge of the principles and practices of health education.
  • Skill in operating a personal computer.
  • Ability to operate audiovisual equipment.
  • Ability to design and produce health promotion and public health educational materials for the media.
  • Ability to develop and deliver health education services.
  • Ability to collect and analyze data.
  • Ability to plan, organize and coordinate work assignments.
  • Ability to utilize computer systems.
  • Ability to provide health education services to others.
  • Ability to communicate effectively.
  • Ability to establish and maintain effective working relationships with others.
  • Ability to understand and apply applicable rules, regulations, policies and procedures.
  • Valid Driver License
  • Must be able to learn and communicate effectively, orally and in writing, in English.

Responsibilities

  • Collaborates with department supervisory staff on development and delivery of outreach activities for health promotion and public health education services to the community.
  • Assesses individual and group health risk status.
  • Conducts client/community surveys to determine the effectiveness of current programs and to assess health promotion needs not currently being addressed in the community.
  • Plans, manages, coordinates and facilitates outreach activities for health promotion and public health education.
  • Provides health care information and serves as a resource specialist to other agencies and groups.
  • Operates and maintains audiovisual equipment, computers, projection panels, teleconferencing equipment and related materials.
  • Collaborates with department supervisory staff on the preparation of health promotion and public health education material.
  • Designs and produces health education media materials, brochures and displays for health fairs, seminars, conferences, articles, flyers, posters, & video creation, etc.
  • Reviews and compiles health promotion and public health education materials developed for use in training programs.
  • Maintains health promotion and public health education library, the resource room and equipment.
  • Prepare purchase requisitions as required for supplies need to support this position.
  • Responsible as a “need to know” employee for security sets relating to active medical records as it relates to performance of assigned duties and responsibility of same.
