About The Position

Community Health Action of Staten Island (CHASI), in partnership with Sun River Health, has developed a collaborative program to enhance care engagement, improve HIV health outcomes, and promote the well-being of people living with HIV/AIDS (PLWH/A) in Queens and Staten Island. The Ryan White Support Services program addresses unmet social service needs that have significantly hindered their access to and engagement in HIV and other essential medical care. The Health Educator, with the support of the Comprehensive Services Coordinator, Community Navigators, and Peer Advocates, will collaborate with the medical and case management teams at the Sun River’s Health Center on Staten Island and Sutphin Health Center in Jamaica, Queens. This program will expand services to Staten Island and establish community-based, non-Medicaid eligible services in Queens to assess, identify, and address unmet social determinants of health (SDOH) needs. The Health Educator will be responsible for facilitating NYS Department of Health-approved HIV Health Education/Risk Reduction curriculum in small groups, food and nutrition counseling/education, and developing an individualized services plan.

Requirements

  • At least 2 years of experience with case management and/or patient navigation REQUIRED.
  • High school diploma or equivalency with related experience and strong socio-cultural identification with the target population or certification as a peer worker required; some college, Associates, or Bachelor's degree preferred.
  • Bilingual (English/Spanish) highly preferred.
  • Computer literacy in Microsoft products: TEAMS, Outlook, etc., and ability to navigate the internet.
  • Knowledge, understanding, and ability to communicate HIV progression, treatments, and adherence strategies.
  • Knowledge and understanding of care management principles and documentation.
  • Can actively listen to feedback and act accordingly.
  • Compose documentation, emails, and other communications that are clear, accurate, and well-organized.
  • Understand and convey information and instructions so that the message accomplishes its intended purpose clearly with colleagues, clients, and external parties.
  • Work harmoniously with others to accomplish objectives, gaining the cooperation and respect of others in the process. Relates effectively with staff, clients, supervisors, and senior management.
  • Show up professionally each day within the context of expectations of their work environment.
  • Work independently following receipt of instructions on a task.

Responsibilities

  • Implement curriculum-based sessions covering basic HIV-related knowledge, treatment education, and prevention strategies at both the Staten Island and Queens sites.
  • Facilitate group and individual sessions promoting positive health outcomes and healthy eating.
  • Provide team-based learning opportunities on the Health Education Curriculum and/or relevant health-related topics.
  • Conduct individual health education sessions for members as they relate to their health care and treatment.
  • Maintain updated case records through the agency’s electronic medical record (EMR) in accordance with Health Resources and Services Administration (HRSA) defined services, agency standards, and regulatory requirements.
  • Assist in enrollment activities including intake assessment and care planning.
  • Lead outreach and case-finding activities to support the promotion of services and program growth.
  • Coordinate logistics for care and treatment including referral, appointment reminders, preparation, and transportation.
  • Collaborate with service providers and provide critical feedback to members of the care team.
  • Identify strengths and challenges for retention in care, adherence, and viral suppression through the assessment process.
  • Develop care plan goals using the SMART framework.
  • Actively participate in conferences, supervision, and learning opportunities.
  • Adhere to standards for documentation requirements, data submission, and the maintenance of charts.
  • Complete administrative tasks such as maintaining MetroCard and incentive logs.
  • Perform all functions in alignment with CHASI’s Mission, Vision, and Core Values.
  • Other duties as assigned.
