About The Position

The Outpatient Nurse Coordinator is a Registered Nurse and a member of the care delivery team. This role is responsible for the comprehensive care of the patient, managing multiple aspects of the patient care package even with minimized patient contact. The Outpatient Nurse Coordinator utilizes independent judgment and maintains continuous communication with the patient, family, physician, and community resources to assist the patient in achieving optimal outpatient health. This role embodies a commitment to the community and the nursing profession.

Requirements

  • Current licensure as a Registered Professional Nurse in the practicing state.
  • CPR Certification.
  • Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state. Hires and current employees working on an out-of-state NLC license who later change their residency to the state where the facility is also located will have 60 days upon changing their residency to apply for licensure within that state.
  • Minimum 3 years nursing experience.
  • Demonstrates knowledge and skills necessary to provide care and/or interact appropriately to the ages of the patients served by his/her assigned unit.
  • Demonstrates knowledge of the principles of growth and development over the life span.
  • Possesses the ability to assess data reflective of the patients status and interpret the appropriate information needed to identify each patients requirements relative to his/her age-specific needs and to provide the care needs as described in the department policy and procedures.

  • CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire.

  • Formulates goals and a plan of care for patients that involves the patient as a partner and considers individual needs holistically.
  • Triages patient situations and coordinates care with the physician.
  • Assists the physician with all aspects of patient care, including assessment, evaluation, and education.
  • Provides care for patients in a global, holistic manner, viewing patient responsibility as continual, not just at the time of office/procedural visits.
  • Accountable for outpatient treatment follow-up necessary for patients to maintain health between visits.
  • Serves as the primary nursing resource to the patient caseload.
  • Facilitates necessary services outside the healthcare facility and interacts with homecare and other support services.
  • Develops and maintains productive working relationships internally and externally by demonstrating accountability for actions, enthusiasm, motivation, and commitment to patients and colleagues.
  • Demonstrates an understanding of cultural differences and holds peers accountable for healthy relationships.

  • Acts as the patient advocate and voice to physicians and physician extenders within the practice.

  • Ensures patient documentation is comprehensive and promotes communication between caregivers.

  • Provides detailed and appropriate teaching to patients and families to guide them through the episode of care in the office setting and via phone/written communication.
  • Serves as the primary nursing liaison for the development of a total care plan to the patient and family.
  • Supports the development of students, new staff, and colleagues.
  • May serve as a preceptor for newly acquired staff.
  • May be responsible for the coordination of staff training and educational activities.
  • Functions as the senior nursing level, assisting in the professional development and maturity of new staff members and ancillary support.
  • Creates an environment of open dialogue, inquiry, and continuous development by asking for feedback and improving practice.
