About The Position

OCES is growing and expanding its services to 11 more towns in the south shore area, including Quincy and Braintree. OCES is dedicated to supporting older adults and individuals with disabilities by providing essential information and coordinating services to promote healthy, safe living, enabling them to remain active and engaged in their communities. The One Care Plan Advocate will work within the Senior Care Options (SCO) department to empower consumers in meeting their Long Term Services and Supports (LTSS) needs through assessment, person-centered care planning, advocacy, and participation in the Interdisciplinary Care Team (ICT). The role aims to promote individualized, cost-effective care plans that enhance quality of life.

Requirements

At least two years of experience in the human services field.
Passion for helping others and desire to make a difference.
Adaptability to changing consumer and/or program needs.

Nice To Haves

Bachelors degree in Social Work or related field highly preferred.
Current licensure if appropriate.

Responsibilities

Conduct comprehensive needs assessments and develop individualized care plans.
Support and advocate for person-centered care planning and provision of LTSS in ways that promote independent living.
Represent the needs of the OCP enrollee by identifying communication and support needs, advocating for the enrollee, and providing education on LTSS to the ICT and enrollee.
Monitor the provision and effectiveness of community services according to the ICP.
Identify community-based alternatives to long-term institutional care.
