Requirements

  • PharmD degree from an accredited school of pharmacy.
  • Active Minnesota Pharmacist License

  • PGY-2 Oncology Residency
  • Board Certification in Oncology Pharmacy

Responsibilities

  • Dispensing medications and pharmaceutical products while adhering to established professional standards and best practices.
  • Collaborating with nursing and medical staff to provide guidance on drug therapy.
  • Supporting oncology services across both IV admixture and oral specialty pharmacy services.
  • Coordinating with physician staff and service providers in planning and implementing patient care plans to ensure the safe and timely delivery of drug therapy.
  • Maintaining thorough and appropriate documentation in the patient clinical chart, including communication with other healthcare professionals.
  • Providing counseling and answering questions from patients and family concerning drug therapy.
  • Instructing patients on the effects of food and alcohol intake and warning of potential side effects.
  • Providing professional drug information and assistance to medical staff.
  • Preparing and dispensing medication orders.
  • Maintaining and evaluating patient profiles and comprehensive drug data files to assess orders for any incompatibilities.
  • Checking all orders for completeness of information, verifying patient information, and proper physician authorization.
  • Ensuring insurance approval before admixing.
  • Admixing drugs as ordered by physicians accurately and timely per department guidelines and preparing all drugs within OSHA and interoffice guidelines.
  • Ensuring correct preparation of all formulas or medications.
  • Checking all prescriptions for completeness, calculating correct dosages, converting between metric and apothecary equivalents.
  • Organizing and directing technician workflow; verifying their preparation and labeling of pharmaceuticals; verifying order entries, charges, and inspections.
  • Providing pharmacological information by answering questions and requests from healthcare professionals and patients.
  • Ensuring adherence to all legal and professional requirements, including those for controlled substances.
  • Consulting with patients to determine pre-existing conditions to assess medication compatibility.
  • Participating in pharmacy teaching and in-service programs.
  • Logging and dating all drugs in use, checking drugs monthly for outdates, and keeping accurate temperature logs.
  • Maintaining accurate logs on all protocol drugs and patient assistance drugs.
  • Assisting with inventory control of medications, ordering supplies and medications, and receiving and shelving medications.
  • Ensuring standard drug supplies are adequately stocked in all departments.
  • Preparing labels, filing, preparing billing, and computing prices.
  • Maintaining confidentiality of all information related to patients and medical staff.
  • Preparing reports and monitoring activities as directed.
