About The Position

The Oncology Pharmacist provides clinical support and consultative services in a complex retail pharmacy operation. The role partners with key stakeholders to advance medication safety, customer service, process improvement, clinical and formulary programs and initiatives, and regulatory compliance. The pharmacist is also a primary resource for developing and educating students, residents, staff, and interdisciplinary care team members. This is a float position rotating among Adult Fairview Ambulatory Infusion Centers, including clinics and surgery center, Ridges, Maple Grove, and Southdale. The Clinics and Surgery Center is the home site.

Requirements

One year of pharmacist experience in a complex pharmacy operation or completion of an ASHP-accredited PGY1 residency program.
Licensed or eligible for licensure as a pharmacist in Minnesota.

Nice To Haves

Completion of an ASHP-accredited PGY1 residency program.
Doctorate of Pharmacy.
Three years of experience in a specialty setting.

Responsibilities

Provide clinical, formulary, and pharmacotherapy expertise and act as a resource to students, staff, and interdisciplinary care team members.
Provide pharmaceutical care to patients and partner with interdisciplinary care team members to ensure medication orders and therapies are appropriate, drug selection and formulary use are correct, and applicable formulary and departmental initiatives are followed.
Provide clinical pharmacy services to patients in the assigned area, including daily patient reviews, monitoring and adjusting therapies, and documenting changes according to department policies.
Be accountable for the distribution and/or preparation of medications, sterile products, compound IV mixtures, and other extemporaneous orders as appropriate.
Consult with and educate patients, staff, and students on drugs.
Execute the function of a staff pharmacist.
Perform other duties as assigned.
