Hartford HealthCare, the largest and most comprehensive healthcare system in Connecticut, has a new growth opportunity for an Oncology Nurse Practitioner or Physician Assistant, to join our cohesive team to provide value-based care to patients in practice-based settings in Hartford Healthcare’s East Region. This role involves providing value-based care in an ambulatory setting, and will be involved in triage and the spectrum of outpatient management including, but not limited to the following: Benign hematology visits, sick/urgent visits, chemotherapy education/teach visits, bone marrow aspirations, overseeing infusion follow up appointments, and ongoing survivorship care. The Hartford HealthCare (HHC) Cancer Institute is a fully certified member of the Memorial Sloan Kettering (MSK) Cancer Alliance, an initiative designed to establish partnerships between MSK and local care providers to facilitate collaboration and provide state-of-the-art, evidence-based cancer care.
