About The Position

We are seeking a compassionate and highly organized Oncology Nurse Navigator to guide patients through the cancer care continuum. This role serves as a key point of contact for patients, families, and the multidisciplinary care team, ensuring timely access to care, coordination of services, and a seamless patient experience. The Oncology Nurse Navigator provides clinical expertise, education, and support from diagnosis through treatment and survivorship, while helping to remove barriers to care.

Requirements

Current Registered Nurse (RN) license in WI
Minimum of 3–5 years of oncology nursing experience
Strong clinical assessment and critical thinking skills
Excellent communication and interpersonal skills
Ability to manage multiple priorities in a fast-paced environment

Nice To Haves

Bachelor of Science in Nursing preferred
Oncology Certified Nurse (OCN) preferred or required within 24 months
Experience in care coordination or navigation
Knowledge of oncology treatment pathways
Familiarity with EPIC
Strong patient advocacy skills

Responsibilities

Coordinate and ensure seamless, evidence-based care across the oncology continuum
Conduct comprehensive assessments and collaborate with the multidisciplinary team to develop individualized care plans
Provide patient and family education, support, and advocacy, addressing clinical and psychosocial needs
Identify and address barriers to care to promote timely access and adherence to treatment
Develop and deliver education materials and outreach programs for patients, staff, and the community
Participate in quality improvement initiatives and maintain clinical expertise to support optimal patient outcomes
