About The Position

Pagosa Springs Medical Center (PSMC) is seeking a compassionate and experienced Oncology Nurse Navigator to support patients throughout the cancer care continuum. The Oncology Nurse Navigator serves as a clinical guide, advocate, and coordinator, helping patients navigate diagnosis, treatment, and follow-up care. This role works closely with providers, infusion staff, and external partners to ensure timely access to care, remove barriers, and improve the overall patient experience. The ideal candidate is an experienced oncology nurse who is passionate about patient advocacy, education, and coordination of complex care in a clinic-based, multidisciplinary environment.

Requirements

  • Graduate of an accredited school of nursing
  • Current Colorado RN license in good standing
  • Current AHA BLS certification
  • Current ACLS certification (or completion within 3 months of hire)
  • Minimum three (3) years of hospital-based RN experience
  • Minimum one (1) year of Oncology experience
  • Strong clinical assessment and critical thinking skills
  • Knowledge of oncology pathophysiology and treatment modalities
  • Proficiency in EHR documentation and oncology management software
  • Ability to work independently in a high-stress, fast-paced environment
  • Excellent communication and patient education skills

Nice To Haves

  • BSN or MSN preferred but not required
  • Infusion experience preferred
  • Oncology infusion experience preferred
  • ONS/ONCC Chemotherapy Certification preferred
  • If not certified, must complete ONS Chemotherapy/Biotherapy Fundamentals within 60 days of hire and pursue certification within 30 months

Responsibilities

  • Lead coordination of care from initial oncology referral through treatment and follow-up
  • Manage incoming oncology referrals
  • Serve as liaison between patients, caregivers, providers, and the care team
  • Facilitate record retrieval and documentation within the electronic health record
  • Connect patients with community resources, financial assistance, transportation, and support services
  • Coordinate imaging, biopsies, surgery, PET scans, and specialty consultations
  • Assist patients with prior authorizations (oral medications, infusion therapy, PET scans)
  • Advocate for patients at all levels of care
  • Practice within RN scope as defined by the State of Colorado
  • Perform comprehensive patient assessments and nursing histories
  • Administer injections, IV therapy, chemotherapy/biotherapy per ONS guidelines
  • Perform IV insertions, PICC line and Port-a-Cath management
  • Perform phlebotomy and therapeutic phlebotomy
  • Administer blood transfusions per protocol
  • Monitor and manage infusion pumps
  • Apply evidence-based symptom management and patient education
  • Follow the Seven Rights of Medication Administration
  • Maintain strict adherence to National Patient Safety Goals
  • Complete accurate documentation in the electronic health record
  • Assist with outpatient infusion services
  • Cross-train to effectively perform RN – Infusion duties
  • Maintain safe handling and administration of chemotherapeutic agents
  • Utilize ONS, NCCN, Lexicomp, and pharmacist resources to ensure patient safety
