On Call RN - 7 On / 7 Off
About The Position
Continuum Hospice is seeking a Full Time On-Call Registered Nurse (RN) to provide exceptional care and support to hospice patients and their families. The On-Call RN will identify patient and family needs, coordinate care, and offer preventative measures and compassionate support. The schedule is 7 days on and 7 days off, covering specific hours from Friday evening to Monday morning, and Monday evening to Friday morning. Requirements
- Current unencumbered license as a Registered Nurse.
- Bachelor's degree in nursing from a program accredited by the National League for Nursing; OR One (1) year experience as a professional nurse within the last three (3) years.
- Computer proficiency in typing and various programs, including a background in EMR.
- Current CPR certification.
Nice To Haves
- Hospice/home health experience preferred but not required.
Responsibilities
- Respond to a variety of patient needs, from urgent to routine, providing unwavering support in critical moments.
- Conduct comprehensive assessments of patient's physical, emotional, social, and spiritual needs.
- Maintain accurate documentation in the patient's home and within agency and regulatory standards.
- Share knowledge and expertise to guide and support families in caring for their terminally ill loved ones.
