About The Position

This role involves implementing, coordinating, and supervising all aspects of patient care delivery for the hospice program. The On-Call Hospice RN will triage incoming calls for referrals or care, implementing appropriate plans such as home visits, professional consultations, or interagency referrals. They will also provide consultation and visits for pain and symptom management, comfort care, HHA orientations/supervisions, Death Pronouncement, and bereavement care. Maintaining ongoing communication with the Interdisciplinary Group (IDG) staff, reporting patient issues/contacts before 8:30 a.m. for evening/night activities, seeking consultation with other agency staff, and participating in Hospice IDG Team and other meetings as requested are key responsibilities.

Requirements

  • Certified in CPR
  • Attends yearly OSHA in-service and completes annual skills validation
  • Identifies knowledge gaps and remedies independently
  • Attends minimum 12 hours of in-services per year, including 6 hours of hospice in-service.
  • Current license to practice as RN in Connecticut.
  • Valid driver’s license
  • Minimum three years clinical experience in the past five years, at least one of which must be palliative care, end of life care, long-term care, or hospice care.
  • An understanding of and commitment to hospice principles and philosophy.
  • Demonstrated ability to manage all aspects of terminal care with expertise in pain and symptom management and hospice nursing.

  • BSN degree preferred.

  • Calls in report before 8:30 a.m. to all appropriate parties on patient issues/contacts during evening/night.
  • Seeks consultation with other agency staff as indicated.
  • Participates in Hospice IDG Team and other meetings, as requested.
