Requirements

Minimum of one year recent home health experience
Minimum of one year recent hospice experience
Graduate of an accredited school of nursing, BSN preferred
Minimum of 2 years nursing experience
Ability to provide own transportation
Current Missouri and IL State Nursing License or eligible to obtain
Proficient in computer skills

Responsibilities

Audits open and closed charts to meet established goals
Notifies staff of corrections that need to be made to documentation and follows up to see that work is completed
Implements QI programs for compliance to meet conditions of participation
Prepares monthly and quarterly QI reports based on identified quality outcomes
Chairs QA/PI Committee
Holds regularly scheduled QA/PI meetings
Tracks patient hospitalizations, infections and falls
Reviews patient plan of care and generates 485 to meet regulatory standards
Reviews patient admission medication list for accuracy on therapy only admissions, following up with client, physician, or family if issues are found
Codes client medical charts per regulation and specific payor policy, maintaining ethical/legal coding practices
Prepares documentation for staff meetings as needed
Orientates new staff to QA/PI process
Provides on-going education to staff
Attends staff meetings and in-services as required
Performs duties of staff nurse on an as needed basis
Serves as the administrative person – on-call as scheduled
