About The Position

The Illinois Department of Public Health is seeking a highly motivated individual to perform a variety of complex and specialized confidential processes regarding vital records. This role involves applying laws, regulations, and agency policies to process delayed birth and death records, amend birth and death records, create new birth records, and seal original files. The position requires independent judgment in making determinations on various important vital records and problems that do not involve deviation from established policies and procedures.

Requirements

  • Knowledge, skill, and mental development equivalent to completion of high school.
  • Two (2) years of office experience.
  • Ability to keyboard accurately at 45 wpm.

Responsibilities

  • Converse with applicants in person, by telephone, or in writing to address a variety of inquiries requiring knowledge of the Illinois Vital Records Act and Code necessary to process requests for vital records, make amendments to birth or death records, and create a new or delayed birth record.
  • Review requests to determine if provisions of the Vital Records Act and Code have been met for eligibility, acceptability, or validity of the case from the applicant’s correspondence, court orders, and Agency policies; amend or create a vital record accordingly.
  • Respond to inquiries, create, and keyboard letters of a technical nature to applicants citing necessary supportive documentation when it is determined that the applicant’s request cannot be processed or completed due to ineligibility or lack of sufficient data.
  • Certify official documents needed to obtain passports, driver’s licenses, social security benefits, probate wills, etc.
  • Amend original birth and death records or create new birth records and seal original files in accordance with the Vital Records Act and Code after review for compliance.
  • Assist with opening, reading, sorting, distributing, and mailing of all correspondence received in the Division of Vital Records, filing and pulling of files, and pulling and copying of original birth records.
  • Perform other duties as required or assigned which are reasonably within the scope of the duties enumerated above.
