About The Position

FOX Rehabilitation is hiring licensed Occupational Therapy Assistants to provide hands-on care to older adults in Riverdale, NY, through its Geriatric House Calls™ model. The role involves delivering individualized treatment plans under the direction of a supervising Occupational Therapist and helping patients improve strength, mobility, confidence, and independence in their homes and senior living communities. OTAs work one-on-one with patients as part of an interdisciplinary team that includes physical therapists, occupational therapists, speech-language pathologists, and Fitness Specialists. FOX offers built-in mentorship, collaborative care planning, and opportunities to develop clinical skills, caseload, and career. Clinicians can build their own schedules and establish lasting relationships with patients. Full-time, part-time, and PRN/FOX Flex employment pathways are available. The New York standard base pay range is $21.63–$36.06 per hour, with opportunities to earn more.

Requirements

Valid Occupational Therapy Assistant license in the state or states of practice, or eligibility to obtain one.
Graduation from an accredited Occupational Therapy Assistant program.
Current CPR certification.
Basic computer skills and proficiency in Microsoft Office.
Ability to meet the physical demands of in-home and community-based care, including lifting, CPR, patient transfers and bed mobility, varied postures and extensive walking, repetitive upper-extremity movements, driving to patient locations, and wearing PPE when required.

Responsibilities

Deliver individualized treatment plans established by the supervising Occupational Therapist, adjusting activities based on patient response and progress.
Instruct, motivate, and safeguard patients during therapeutic exercises and functional activities, while observing and tracking progress.
Document visits accurately and on time in FOX’s EMR system, following FOX’s Clinical Documentation standards.
Stay connected with patients, powers of attorney (POAs), and community referral sources, and help coordinate coverage when needed.
