About The Position

At Dublin Post Acute, we are committed to providing exceptional skilled nursing and rehabilitation services that help our residents regain independence and enhance their quality of life. Our interdisciplinary team delivers compassionate, patient-centered care in both short-term rehabilitation and long-term care settings. Dublin Post Acute is seeking a dedicated Occupational Therapy Assistant to join our in-house rehabilitation team on a PRN (as-needed) basis. Under the supervision of a licensed Occupational Therapist, the COTA will implement individualized treatment plans and assist residents in improving their ability to perform activities of daily living (ADLs) while promoting functional independence.

Requirements

Current Certified Occupational Therapy Assistant (COTA) license in the State of Ohio.
Associate degree from an accredited Occupational Therapy Assistant program.
Strong documentation, organizational, and time management skills.
Excellent communication and interpersonal skills.
Compassionate, patient-focused approach to care.

Nice To Haves

Previous experience in a Skilled Nursing Facility (SNF), Long-Term Care (LTC), or rehabilitation setting preferred.

Responsibilities

Implement treatment plans developed by the Occupational Therapist.
Assist residents with activities of daily living, including bathing, dressing, grooming, and toileting.
Guide residents through therapeutic exercises and functional activities.
Monitor, document, and report patient progress accurately and promptly.
Educate residents and family members on adaptive techniques, safety strategies, and assistive equipment.
Maintain a safe, clean, and supportive therapy environment.
Collaborate with the interdisciplinary care team, including Physical Therapy, Speech Therapy, Nursing, and Social Services, to provide comprehensive patient care.
