About The Position

Innovate Rehab and Wellness is hiring a Licensed PRN Certified Occupational Therapy Assistant (COTA) to provide occupational therapy services in a skilled nursing facility. The role supports high-quality, patient-centered care, helping residents regain independence and improve their quality of life through therapeutic interventions under the supervision of an Occupational Therapist. This is an as-needed PRN position at Brookfield Health Care Center in Brookfield, Missouri, with flexible scheduling. The role emphasizes one-on-one patient care, with group or concurrent treatments comprising less than 1% of treatment. Requirements

Associate’s degree from an accredited occupational therapy assistant program.
Current NBCOT certification.
Valid state license as an Occupational Therapy Assistant.
Strong communication and interpersonal skills.

Commitment to patient-centered care and rehabilitation excellence.

Responsibilities

Implement treatment plans created by the Occupational Therapist.
Provide direct patient care in accordance with state and facility guidelines.
Monitor and document resident progress and report changes to the Occupational Therapist.
Educate residents and caregivers on adaptive techniques and equipment.
Collaborate with nursing, physical therapy, speech-language pathology, and other healthcare staff.
Maintain accurate and timely documentation in the electronic medical record.
Participate in care planning and discharge planning.
Ensure compliance with HIPAA, CMS, and applicable regulations.
