About The Position

Empath Home Health, a member of Empath Health, is seeking a caring and motivated PRN Home Health Certified Occupational Therapy Assistant (COTA) to provide home visits in Marion, Lake, and Sumter counties. Empath Home Health provides skilled nursing and therapy services to help patients recover and maintain independence at home, as well as personal care support for daily needs.

The COTA provides occupational therapy services under the supervision of the Occupational Therapist/Director, using therapeutic self-care and creative activities to improve patients’ independence and physical and mental functioning. The role also evaluates equipment needs and prepares timely clinical and progress notes documenting services, activities, and patient condition.

The schedule is PRN, with 5–10 or more home health visits per week. Empath Health highlights flexible scheduling, manageable caseloads, interdisciplinary support, mileage reimbursement, and the possibility that some roles may expand to full-time as the market grows.

Requirements

• Proof of completion of an Occupational Therapy Assistant program.
• State licensure as an occupational therapy assistant.
• At least one year of experience as a licensed Occupational Therapy Assistant.

Responsibilities

• Provide occupational therapy services according to the plan of care and under the supervision of the Occupational Therapist/Director.

• Prepare clinical and progress notes documenting services provided, patient activities, and condition, and submit them in a timely manner.
