About The Position
RallyX is seeking a licensed Occupational Therapy Assistant to provide home health services to patients in Duval County. This role involves delivering occupational therapy services and assessments, guiding patients through therapeutic care, assisting in the development of patient care plans, and preparing progress notes.
Requirements
- Graduated after successful completion of an Occupational Therapy Assistant education program accredited by: The Accreditation Council for Occupational Therapy Education (ACOTE) of the American Occupational Therapy Association, Inc. (AOTA), or its successor organizations.
- Eligible to take, or successfully completed, the entry-level certification examination for Occupational Therapy Assistants developed, and administered, by the National Board for Certification in Occupational Therapy, Inc. (NBCOT)
- Licensed to practice within the state of Florida
- CPR certification required (Online certification not accepted)
- At least one year of institutional experience
- Reliable Transportation

Responsibilities
- Provide occupational therapy services and assessments to patients in their homes
- Guide and instruct patients through prescribed therapeutic care
- Assist in the implementation and development of patient care plans
- Prepare progress notes following visits for patients’ charts
