Requirements

  • Bachelor’s degree in an occupational therapy curriculum accredited by ACOTE standards.
  • Initial registration from the National Board for Certification in Occupational Therapy.
  • Valid and unencumbered occupational therapy license as required in the state of practice.
  • Valid driver’s license or reliable, independent transportation to and from the patient’s residence.

Responsibilities

  • Provide outpatient rehabilitation services in an in-home setting for the older adult population.
  • Evaluate and treat patients in their home environment.
  • Organize and conduct occupational therapy programs.
  • Direct patient participation in selected tasks to restore, reinforce, and enhance performance.
  • Facilitate the learning of skills and functions essential for adaptation and productivity.
  • Supervise Occupational Therapy Assistants, temporary licensees, students, and support personnel as required.
  • Adhere to department and patient treatment schedules, clinical and billing documentation requirements, and work-related meetings.
  • Attend and contribute to patient care, staffing conferences, and other related meetings.
  • Make referrals to other disciplines and agencies to address comprehensive patient needs.
  • Consult with other members of the rehabilitation team and community physicians.
  • Adapt program and/or plan of care according to individual patient needs.
  • Promote a safe environment and enforce the elimination of fire and safety hazards.
  • Order supplies and equipment as necessary.
  • Clean, maintain, identify, and report the need for repair of equipment.
  • Ensure patients and families receive the highest quality of service in a caring and compassionate atmosphere.
  • Screen, examine, and evaluate patients, including history, systems review, and application of appropriate tests and measures.
  • Synthesize information and interpret examination findings to establish a diagnosis, identify impairments, determine the predicted level of improvement and time required, identify precautions/contraindications, and design a plan of care.
  • Develop appropriate treatment goals and methods in collaboration with the patient and caregivers.
  • Implement the occupational therapy treatment plan and complete all related documentation and record keeping.
  • Provide services including application of therapeutic techniques in areas such as cognitive skills, muscle strength, coordination, endurance, mobility, perceptual abilities, sensory awareness, sitting and standing tolerance, balance, activities of daily living, joint protection, work simplification, orientation, physical agent modalities, low vision, falls risk management, medication management, health and wellness, community integration and reintegration.
  • Utilize case management skills in the home and community space.
  • Design, fabricate, fit, and/or apply adaptive devices, and train patients in the use of adaptive devices including orthotics and prosthetics.
  • Select appropriate activities suited to the individual’s physical and mental capacity, intelligence level, and interest.
  • Instruct patients in the skills and techniques required for participation in functional activities.
  • Evaluate patient progress, attitudes, and behavior as related to rehabilitative potential.
  • Perform routine reexamination as needed/required to modify/progress plan of treatment.
  • Perform other related duties as required.
