Requirements

  • Bachelor’s degree in an occupational therapy curriculum accredited by ACOTE standards.
  • Initial registration from the National Board for Certification in Occupational Therapy.
  • Valid and unencumbered occupational therapy license as required in the state of practice.
  • Valid driver’s license or reliable, independent transportation to and from the patient’s residence.

Responsibilities

  • Provide outpatient rehabilitation services in an in-home setting for the older adult population.
  • Evaluate and treat patients in their home environment, which could be a private residence or a patient's room in an independent living facility.
  • Organize and conduct occupational therapy programs to facilitate rehabilitation.
  • Direct patient participation in selected tasks to restore, reinforce, and enhance performance.
  • Facilitate the learning of skills and functions essential for adaptation and productivity to diminish or correct pathology.
  • Provide a flexible schedule and work environment in various settings.
  • Supervise Occupational Therapy Assistants, temporary licensees, students, and support personnel as required by state licensure and professional standards.
  • Adhere to department and patient treatment schedules, timely clinical and billing documentation, and work-related meetings.
  • Attend and contribute to patient care, staffing conferences, and other related meetings.
  • Make referrals to other disciplines and agencies to address comprehensive patient needs.
  • Consult with the rehabilitation team, including community physicians, to determine the most appropriate therapeutic regimen.
  • Adapt programs and/or plans of care based on individual patient needs.
  • Promote a safe environment and enforce the elimination of fire and safety hazards.
  • Order necessary supplies and equipment.
  • Clean, maintain, identify, and report the need for repair of equipment.
  • Prioritize patient service by ensuring the highest quality of care in a compassionate atmosphere, recognizing individual needs and rights.
  • Screen, examine, and evaluate patients, including history, systems review, and application of tests and measures.
  • Synthesize information and interpret examination findings to establish a diagnosis, identify impairments, determine predicted improvement and time required, identify precautions/contraindications, and design a plan of care.
  • Develop appropriate treatment goals and methods in collaboration with patients and caregivers.
  • Implement the occupational therapy treatment plan and complete all related documentation and record keeping.
  • Provide services including, but not limited to, therapeutic techniques for cognitive skills, muscle strength, coordination, endurance, mobility, perceptual abilities, sensory awareness, sitting and standing tolerance, balance, activities of daily living, joint protection, work simplification, orientation, physical agent modalities, low vision, falls risk management, medication management, health and wellness, and community integration/reintegration.
  • Utilize case management skills in home and community settings.
  • Design, fabricate, fit, and/or apply adaptive devices, and train patients in their use, including orthotics and prosthetics.
  • Select appropriate activities suited to the individual's physical and mental capacity, intelligence level, and interest.
  • Instruct patients in skills and techniques for functional activities.
  • Evaluate patient progress, attitudes, and behavior related to rehabilitative potential.
  • Perform routine reexamination as needed to modify/progress the plan of treatment.
  • Perform other related duties as required.
