Requirements

  • Active Minnesota OTR/L license.
  • Strong clinical judgment and problem-solving ability.
  • Confidence working independently in a field-based setting.
  • Passion for delivering outcome-driven, patient-centered care.
  • Reliable transportation for home visits.

Responsibilities

  • Perform in-depth in-home evaluations focused on function, safety, and independence.
  • Design customized treatment plans based on real-life environments.
  • Address ADLs, mobility, cognitive function, and home safety barriers.
  • Recommend and implement adaptive equipment and environmental modifications.
  • Educate patients and caregivers to drive lasting outcomes.
  • Document and collaborate with an interdisciplinary team.
