About The Position

Elara Caring is seeking a PRN Occupational Therapist to provide personalized home health care to patients in Warwick, Rhode Island. The role helps patients regain independence and participate in everyday activities through evaluation, individualized treatment, caregiver education, and collaboration with the care team. Requirements

Current Rhode Island Occupational Therapist license.
At least 1 year of occupational therapy experience.
Current CPR certification through the American Heart Association or American Red Cross.
Strong communication skills and a commitment to quality patient care.
Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
Valid driver's license and acceptable driving record, or reliable transportation to travel between patient homes and assigned work locations.

Responsibilities

Evaluate patients’ functional abilities and develop individualized treatment plans.
Help patients improve daily living skills, strength, coordination, and safety.
Recommend adaptive equipment and home modifications.
Teach patients and caregivers techniques that support independence between visits.
Monitor progress and adjust treatment plans as needed.
Collaborate with physicians and the interdisciplinary care team.
Complete accurate, timely clinical documentation.
Supervise occupational therapy assistants as appropriate.
