About The Position

Lakeshore Manor Nursing & Rehabilitation Center, part of Volare Health, is seeking a compassionate and motivated Occupational Therapist for a flexible PRN role providing primarily weekend coverage for resident evaluations. The Occupational Therapist evaluates residents to determine therapy needs, develops individualized treatment plans, and provides skilled occupational therapy services in accordance with physician orders and established care plans. The role supports functional improvement, rehabilitation outcomes, and residents’ quality of life.

Requirements

Current Occupational Therapist license in good standing, as required by the state of Louisiana.
Strong clinical assessment, treatment planning, and problem-solving skills.
Effective communication and collaboration skills.
Ability to maintain accurate and timely documentation.
Commitment to providing compassionate, resident-centered care.
Availability to provide PRN weekend coverage.

Responsibilities

Conduct occupational therapy evaluations and assess residents’ functional abilities, needs, and rehabilitation potential.
Identify residents who may benefit from occupational therapy intervention.
Develop individualized treatment plans and measurable goals based on each resident’s needs.
Provide skilled occupational therapy services in accordance with physician orders and established treatment plans.
Monitor resident progress and adjust interventions as appropriate.
Collaborate with nursing, rehabilitation, and interdisciplinary team members to support quality resident care.
Participate in discharge planning and make recommendations to promote safe transitions and continued functional progress.
Maintain accurate, timely clinical documentation, treatment records, and billing information.
Meet established productivity expectations while maintaining quality and compliance standards.
Perform other duties as assigned.
