About The Position

Ignite Rehab, operating as part of Metro Physical & Aquatic Therapy, is seeking a motivated and passionate per diem Occupational Therapist to provide in-home care. This is an opportunity to take the next step forward in your career within a growing organization in a progressive, supportive, and innovative team. All levels of experience are encouraged to apply, and training will be provided for the right new graduate. The role offers flexibility and autonomy, allowing you to see patients any 7 days of the week, with a focus on patient care and a quick EMR system (no Oasis required).

Requirements

  • Occupational Therapist Program Graduate
  • Licensed Occupational Therapist in the state of New Jersey or eligible.
  • Mandatory Certifications: First Aid, AED, & CPR Certifications.
  • Ability to travel to patients’ homes within the assigned geographic territory
  • Strong communication skills between patients, support staff, and management, including timely responses via text, email, phone, etc.

Nice To Haves

  • Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.

Responsibilities

  • Accept and treat patients within your designated geographic area
  • Provide 1:1 treatment
  • Evaluate patients and develop individualized treatment plans based on physician orders and patient needs
  • Deliver care that aligns with professional standards, agency policies, and the prescribed plan of care
  • Educate family members, home health aides, and other caregivers on proper therapy techniques
  • Plan, and carry out occupational therapy programs to maintain, improve or restore physical function, relieve pain, and prevent occupational dysfunction in patients.
  • Perform and document an initial exam, evaluating the data to identify problems and determine a diagnosis prior to intervention.
  • Provide timely and accurate documentation for each visit
  • Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit
  • Willingness to treat geriatric patients.
