About The Position
Heralds Home Health is seeking compassionate, dependable, and experienced Occupational Therapists to provide skilled therapy services to patients in their homes. The per-diem role offers flexible scheduling, competitive per-visit compensation, and the opportunity to accept visits based on availability. Therapists may supplement current income or build a consistent patient caseload, with support from clinical and scheduling teams and opportunities for ongoing patient assignments. The work focuses on helping patients improve independence and quality of life through individualized occupational therapy. The position involves one-on-one patient care in the home and coordination with physicians, nurses, and other therapists.
Requirements
- Active California Occupational Therapist license.
- Strong clinical assessment and treatment skills.
- Excellent communication and time-management skills.
- Reliable transportation.
- Ability to work independently in the field.

- Home health experience preferred.
Responsibilities
- Conduct comprehensive occupational therapy evaluations.
- Develop individualized treatment plans.
- Provide skilled occupational therapy services in patients’ homes.
- Help patients improve independence with activities of daily living (ADLs).
- Assess home safety and recommend adaptive equipment.
- Educate patients and caregivers on therapeutic techniques.
- Coordinate care with physicians, nurses, and other therapists.
- Complete accurate and timely clinical documentation.
