About The Position

BAYADA Home Health Care is seeking an experienced Occupational Therapist for a part-time role making home health visits to adult and geriatric patients in Dartmouth, Westport, Fall River, and surrounding towns. The therapist will be an integral member of a multidisciplinary team providing rehabilitative care and skilled nursing so clients can receive medical care at home. Prior home health experience is not required. The position involves evaluating patients’ functional, cognitive, psychological, and motor performance; creating and revising individualized intervention and care plans; assessing home environments and equipment needs; educating patients and their representatives; and documenting care using a touch pad tablet. Base pay is $62.00–$67.00 per point, depending on qualifications.

Requirements

Current Massachusetts Occupational Therapist license.
At least one year of recent work experience as an Occupational Therapist.

Responsibilities

Make home visits to clients in designated geographic territories.
Evaluate patients’ functional status, including muscle function, endurance, visual coordination, communication skills, and ADL and IADL performance.
Assess cognitive, psychological, and motor aspects of performance and develop individualized intervention plans.
Evaluate home environments for hazards or barriers to independent living.
Identify equipment needs and environmental adaptations to improve client outcomes.
Continually assess and revise occupational therapy care plans and participate with nursing in multidisciplinary care planning, as appropriate.
Educate and instruct patients, family members, or other representatives in rehabilitative care and activities that support health, safety, and independent living.
Document observations, interventions, evaluations, and services provided using a touch pad tablet.
