About The Position

Adaptive is building an AI-native home healthcare platform that enables clinicians to focus on delivering best-in-class care for their patients by leveraging software and innovative support solutions. We’re building our team in Detroit with plans to quickly develop an exceptional reputation of delivering excellent patient care. The mission of this role is to deliver excellent home health care to our patients in Detroit, and set the clinical bar, the documentation standard, and the referral-source trust that our market will be built on. Objectives include carrying a high-quality caseload, owning patients' outcomes by driving low avoidable-hospitalization rates and strong patient-experience scores, participating in case conferences/team huddles to improve quality, and being a referral-source ambassador. The role also involves setting the standard for charting, cadence, and judgment as the pod grows.

Requirements

  • Active Michigan OT license (in good standing)
  • Strong clinical judgment and comfort working independently in the field
  • Comfort escalating changes in condition to RN/MD and documenting rationale clearly
  • Strong communication and documentation
  • Reliable follow-through on orders, frequencies, and patient goals
  • Valid driver’s license, auto insurance, and reliable transportation and comfort with daily travel across assigned territory.
  • Able to carry/transport basic therapy equipment as needed
  • Competency in core home health OT scope (ADL/IADL training, home safety, adaptive equipment, caregiver education)

Nice To Haves

  • Prior home health experience
  • Homecare Homebase (HCHB) experience (or similar home health EMR)
  • Deeper experience in high-signal OT populations (neuro, CVA, fall risk, dementia/caregiver training)
  • Strong organization and time management

Responsibilities

  • Complete in-home OT evaluations and deliver skilled OT visits.
  • Build and execute a clear plan of care focused on ADLs/IADLs, safety/fall prevention, and functional independence.
  • Recommend and train on adaptive equipment and home modifications as needed.
  • Coordinate with nursing and the broader care team when condition or needs change.
  • Document promptly and thoroughly in Homecare Homebase (HCHB) — defensible, efficient, and consistent.
