About The Position

Shadelands Post Acute is seeking a compassionate and skilled Occupational Therapist (OT) to join our dedicated in-house rehabilitation team. Requirements

• Current and active California Occupational Therapist (OT) license
• Strong clinical assessment and treatment-planning skills
• Excellent communication and interpersonal skills
• Ability to work effectively within an interdisciplinary team
• Commitment to compassionate, resident-centered care

Responsibilities

• Evaluate residents and develop individualized occupational therapy treatment plans
• Provide skilled OT interventions focused on ADLs, functional mobility, safety, and independence
• Monitor resident progress and modify treatment plans as appropriate
• Supervise and provide clinical guidance to COTAs
• Collaborate with nursing, physicians, therapy professionals, and other interdisciplinary team members
• Participate in care planning and discharge planning
• Educate residents, families, and caregivers on treatment strategies and techniques
• Maintain accurate, timely, and compliant clinical documentation
