About The Position
The Occupational Therapist is responsible for directly, supervising, evaluating, and providing occupational therapy services to patients in their homes as prescribed by the attending physician. This role involves instructing patients and families in daily living activities, establishing household management routines, and educating them on the use of prosthetic, orthotic, and assistive devices, as well as appropriate adaptive equipment. The therapist will manage patient falls risk through evaluation, care planning, education, and interdisciplinary communication. Responsibilities also include recording patient responses and changes in condition, communicating with other health team members, preparing clinical/progress notes, attending case conferences and staff meetings, and directing or assisting with home health aide service delivery. Requirements
- Graduate of an accredited Occupational Therapy program.
- Current, valid OT license issued by NYS Department of Education.
- One year of practical experience as a licensed occupational therapist.
Responsibilities
- Direct, supervise, evaluate and provide occupational therapy services to patients in the home as prescribed by the attending physician.
- Instruct patients and families in the activities of daily living.
- Establish household management routines.
- Instruct patients and their families in the use of prosthetic, orthotic and assistive devices.
- Educate patients and caregivers in the use of appropriate adaptive equipment.
- Manage the falls risk of patients through proper evaluation, care planning, patient/family education, and communication with other clinical disciplines.
- Record and report to the physician the patient’s response to the occupational therapy program or any changes in the patient’s condition through phone contact or periodic written summaries.
- Communicate with other health team personnel involved with the care of the patients.
- Prepare clinical/progress notes on the day of the visit, which are incorporated in the clinical record.
- Attend case conferences and staff meetings.
- Direct or assist with the management of home health aide service delivery.
- Other duties as assigned.
