About The Position

As an Occupational Therapist in Home Health, you’ll be at the heart of our community, delivering compassionate and personalized occupational therapy services. Whether providing direct client care, developing individualized treatment plans, you’ll play a key role in enhancing clients' independence, safety, and overall quality of life.

Requirements

  • Current Occupational Therapist (OT) license in the state of Minnesota.
  • CPR certification within 6 months of hire.
  • Valid driver’s license and access to an insured personal vehicle.
  • Willingness to float across regions as needed.
  • Ability to pass state mandated background check.
  • Strong clinical skills in occupational rehabilitation, skilled therapy services, knowledge of evidence-based occupational therapy practices.
  • Proficiency in reading, writing, and speaking English to communicate effectively with clients, families, and team members.
  • Physical capability to perform all job duties.

Nice To Haves

  • Experience in home health, long-term care, or rehabilitation settings preferred.

Responsibilities

  • Provide Direct Care – Evaluate clients by obtaining and interpreting subjective and objective data. Develop personalized treatment plans with short-term and long-term goals, ensuring they align with physician orders. Maintain high standards of care while adapting to different environments and client needs.
  • Maintain Accurate Client Records and Billing – Document all services accurately and timely using Vivie’s EMR system. Demonstrate a thorough understanding of all payer sources and authorization requirements. Document collaboration with COTAs and perform supervisory visits for HHAs as per regulatory guidelines.
  • Communicate Effectively and Collaboratively – Maintain clear and compassionate communication with clients, families, referral sources, physicians, and the interdisciplinary team. Utilize exceptional verbal and written communication skills to ensure cohesive and effective care delivery.
  • Ensure Regulatory Compliance – Complete and document supervisory visits with OTAs and CNAs in accordance with state and regulatory guidelines.
