About The Position

Empath Home Health, a member of Empath Health, is seeking a compassionate PRN Occupational Therapist to provide individualized home health care in Hernando County, including Brooksville, Spring Hill, Weeki Wachee, and surrounding areas. Visits may also be available in Pasco or Hillsborough counties. The schedule is PRN, with 5–15+ home health visits per week.

The Occupational Therapist provides skilled therapy to help patients recover and maintain independence at home. The role includes assessing patients, developing and updating individualized plans of care, delivering treatments, educating patients and families, and collaborating with an interdisciplinary team. Empath Home Health provides skilled nursing and therapy services, as well as personal care support, as part of a connected continuum of care.

Requirements

• Active Florida OT license.
• At least 1 year of experience as a licensed Occupational Therapist.
• Strong assessment, critical-thinking, and patient-education skills.

Responsibilities

• Perform comprehensive patient assessments, including OASIS assessments at required intervals.
• Develop, initiate, and revise individualized plans of care as patient needs evolve.
• Deliver skilled occupational therapy services and treatments as outlined in the plan of care.
• Counsel and educate patients and families on disease processes, self-care techniques, and prevention strategies.
• Collaborate with the interdisciplinary team to ensure seamless, holistic care.
