About The Position
Sage Connections, a woman- and therapist-owned company, is seeking a W-2 Occupational Therapist (OT) to provide K–12 school-based services for the 2026-2027 school year. This role is ideal for both experienced OTs and those new to the field, offering support from a team that understands the profession. The position is located onsite in Elgin, IL, with a Monday-Friday schedule from approximately 7:30 AM to 3:30 PM, totaling 37.5 hours per week. The contract dates are August 2026 to May 2027, with potential for renewal.
Requirements
- Master's degree in Occupational Therapy
- NBCOT certification (OTR)
- Current Illinois Occupational Therapy license (IDFPR)
- Illinois Professional Educator License (PEL) with School Support Personnel endorsement in Occupational Therapy (or eligibility to obtain)
- Strong communication and collaboration skills
Nice To Haves
- School-based experience preferred, but not required
Responsibilities
- Provide direct and consultative occupational therapy services aligned with student IEPs
- Conduct evaluations and assessments to determine OT needs
- Develop individualized treatment plans and goals
- Collaborate with teachers, families, and school staff to support student participation
- Document services and progress accurately and in a timely manner
- Participate in IEP meetings and contribute to educational planning
