About The Position

Rehab Without Walls Neurorehabilitation is redefining rehabilitation by bringing the outpatient experience directly to the individual. Our Home & Community–based care model allows patients to recover in the environments where life actually happens while giving clinicians the freedom, support, and collaboration they value most. We are seeking an experienced Occupational Therapist for a flexible PRN, W2 role (not 1099) in San Jose, CA. This opportunity offers autonomy, professional stability, and the chance to make a meaningful, real-world impact. We are looking for an experienced Occupational Therapist interested in serving as a primary therapist from evaluation through discharge. Availability for approximately 15–20 hours per week across 3 days, with consistent client relationships. Passionate about functional, real-world rehabilitation delivered in homes and community settings. Motivated to help clients transition back to meaningful daily life, including managing and picking up prescriptions, returning to driving, transitioning back to work, and participating in activities like yoga, light hiking, and community events. This role is for a clinician who thrives with autonomy while valuing collaboration with an interdisciplinary team of OTs, PTs, SLPs, LCSWs, and other specialists. We don’t limit recovery to clinic walls. By bringing outpatient-level therapy to our clients, we help them practice skills where they matter most, leading to stronger independence, better outcomes, and deeper therapeutic relationships. Unlike 1099 roles, this position is W2 PRN, offering organizational and clinical support, payroll consistency and liability coverage, and access to training, leadership, and collaborative care. Requirements

  • Minimum of a bachelor’s degree in Occupational Therapy from a college or university with an accredited Occupational Therapy program
  • Current, unrestricted license as an Occupational Therapist by state in which practicing
  • Current CPR Certification
  • A minimum of one year’s work experience as an Occupational Therapist
  • Demonstrates knowledge of rehabilitation techniques related to complex neurological diagnoses preferred
  • Communicates effectively and professionally in verbal and written interactions
  • Ability to lift 50 pounds
  • Moving, lifting, or transferring of patients which may involve lifting of up to 100 pounds following safety procedures
  • Duties require fine motor skills, visual acuity, and walking/ standing for extended periods
  • Additional physical requirements include: pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently at times
  • A health screen or examination may be required prior to assignment and periodically thereafter, depending on specific location policy, local and state regulations to verify employee is physically capable of performing assigned duties with or without reasonable accommodations

Responsibilities

  • Perform evaluations and develop treatment plans
  • Regularly re-assess effectiveness of treatment plans, attend staffing meetings and family conferences
  • Perform discharge evaluations, make appropriate recommendations for home and community safety, and provide referral and education resources
  • Communicate patient’s needs and progress to the treatment team, physician, person receiving services and family members
  • Communicates with other disciplines to ensure collaboration, coordination of care and enhance patient outcomes
