About The Position

Iowa Home Care is seeking a passionate and skilled Occupational Therapist (OT) for a Part-Time position in home health. The role involves providing in-home evaluations, treatments, and education to help patients improve their ability to safely and independently perform daily activities. The OT will collaborate with a multidisciplinary team, including nurses, physical therapists, and speech therapists, to ensure comprehensive, patient-centered care. This position offers flexible scheduling and the opportunity to make a significant difference by helping Iowans remain safely in their homes.

Requirements

  • Licensed to practice as an Occupational Therapist in the state of Iowa.
  • Minimum of two years of experience in occupational therapy.
  • Strong communication, organizational, and problem-solving skills.
  • Valid driver’s license with reliable transportation.

  • Community/home health experience preferred.

Responsibilities

  • Conduct comprehensive in-home occupational therapy evaluations.
  • Develop and implement individualized treatment plans focused on improving activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
  • Provide therapeutic interventions to improve upper extremity strength, coordination, cognitive skills, and functional independence.
  • Train patients in adaptive techniques to enhance safety and independence with bathing, dressing, grooming, toileting, and meal preparation.
  • Recommend and educate on adaptive equipment and home modifications to improve safety and accessibility.
  • Educate patients and caregivers on home programs, energy conservation techniques, fall prevention strategies, and equipment use.
  • Collaborate with nurses, physical therapists, speech therapists, social workers, and case managers to coordinate care.
  • Document all patient visits, progress notes, and care plans in compliance with agency and regulatory standards.
  • Monitor patient progress and adjust treatment plans as needed.
  • Communicate changes in condition or concerns to the interdisciplinary care team.
  • Promote independence and ongoing improvement through compassionate, patient-centered care.
