About The Position

Join Iowa Home Care as a Part-Time Occupational Therapist (OT) in Fort Dodge, IA. We believe care is best delivered at home. As an OT, you will provide in-home evaluations, treatments, and education to help patients improve their ability to safely and independently perform daily activities. You will collaborate with a multidisciplinary team, including nurses, physical therapists, and speech therapists, to ensure comprehensive, patient-centered care. We offer both full-time and part-time positions.

Requirements

  • Licensed to practice as an Occupational Therapist in the state of Iowa.
  • Minimum of two years of experience in occupational therapy
  • Strong communication, organizational, and problem-solving skills.
  • Valid driver’s license with reliable transportation.

  • Community/home health experience preferred.

Responsibilities

  • Conduct comprehensive in-home occupational therapy evaluations
  • Develop and implement individualized treatment plans focused on improving activities of daily living (ADLs) and instrumental activities of daily living (IADLs)
  • Provide therapeutic interventions to improve upper extremity strength, coordination, cognitive skills, and functional independence
  • Train patients in adaptive techniques to enhance safety and independence with bathing, dressing, grooming, toileting, and meal preparation
  • Recommend and educate on adaptive equipment and home modifications to improve safety and accessibility
  • Educate patients and caregivers on home programs, energy conservation techniques, fall prevention strategies, and equipment use
  • Collaborate with nurses, physical therapists, speech therapists, social workers, and case managers to coordinate care
  • Document all patient visits, progress notes, and care plans in compliance with agency and regulatory standards
  • Monitor patient progress and adjust treatment plans as needed
  • Communicate changes in condition or concerns to the interdisciplinary care team
  • Promote independence and ongoing improvement through compassionate, patient-centered care
