The Occupational Therapist (OT) - Home Health position in Charlotte is a full-time role with day shifts. This position is responsible for the daily management of an assigned patient caseload, ensuring timely care and documentation to meet patient needs as prescribed in the established plan of care and per physician orders. The OT will collect data from patient assessments, evaluate functional abilities for activities of daily living, and develop a plan of care in conjunction with the patient's physician. They will perform OT services, direct care coordination, and supervise COTAs and HHAs. The role involves designing, developing, and fitting patients with orthotic devices and adaptive equipment, instructing family members and staff in therapy methods, and observing/reporting patient condition changes to the physician. Maintaining communication with the multidisciplinary team, incorporating goals into therapy, and providing clear, timely documentation are key responsibilities. The OT must adhere to productivity standards and perform ordered treatments in accordance with state regulations and agency policies. This role requires minimal supervision for delegating care elements and promotes shared governance and research to improve clinical outcomes. The OT is expected to attain knowledge and competence reflecting current practice, integrate evidence and research findings, contribute to quality practice, communicate effectively, demonstrate leadership, collaborate with patients and families, evaluate their own practice, and utilize resources effectively for safe, effective, and financially responsible services.
